COMPUTER/LAPTOP RESPONSIBILITY FORM

The undersigned, an employee of [Firm Name], USA, the employer of said employee, hereby acknowledges that he/she understands that I am not to leave the computer/laptop and/or printer in a vehicle, or unsecured at a client location, and that to do so would be considered negligent.  I further understand that I am responsible for immediately notifying the proper authorities and management if my computer and/or printer should be missing.

In addition, I acknowledge that I have read and understand the Computer Software Policy stated on Page XX and XX of the [Firm Name] Employee Handbook.  I understand that under no circumstances am I to install and/or remove any software from [Firm Name] computers.  Any changes in software should be handled by the [Firm Name] Computer Department.  I also understand that violation of [Firm Name] Computer Software Policy will be grounds for immediate dismissal.
