               1. Send new employee packet, [Firm Name] pin, [Firm Name] portfolio & [Firm Name] plaque to appropriate office manager.  
                         
  2. Add to Time & Billing.                  
    Set up the I.D. number in the T&B program.              
    Email I.D. number to appropriate person.              
    Get budget hours from office manager and enter into T&B.          
    Get pay rate from office manager for payroll.            
                         
               3. Send email to all administrative staff noting name and when they are starting.

    Notify [Name] of new employee if he/she is a CSR.            
                         
  6. Enroll new employee into all insurances.               
    Mail Cobra Continuation Coverage Notice (RRR)            
    Mail the enrollment form to provider (ATTN:  Enrollments), keep a copy for the employee file.    
    Add to (firmdata) Insurance Census.              
    Send employee their insurance card(s).              
    Mail the LTC election form to provider, and put a copy in the provider workfile and wait for acceptance letter.  
    Put the LTD election card in the provider workfile and enroll after 30 day grace period.      
    Make a copy of the life ins application & put in AICPA workfile, mail original to AICPA for coverage.    
    If we receive a proof of insurance from previous provider, fax to Anthem, mail original and keep a copy in emp file.  
    Give copy of 401K enrollment to Pam K. w/my initials-top right hand corner.        
    Send email to Bobbie H. to add to birthday list.            
    Set up benefits in T & B.                
    Place name and date on Awards program-Length of Service Sheet        
                         
               7.  Add to Emp list with codes.xls, Year End Book & Emp Benefit Adj Dates Sheet        
                         
  8. Set up in payroll with employee #.
